Case Manager RN
The Case Manager RN is responsible for coordinating patient care transitions and ensuring optimal resource utilization within the hospital setting. This role involves collaborating with interdisciplinary teams to facilitate smooth patient progression through different levels of care while addressing complex clinical and social needs.
Responsibilities:
- Review and interpret physician care plans to assess patient needs and risks.
- Coordinate care transitions by developing comprehensive discharge plans involving patients, families, and healthcare providers.
- Monitor patient progress, documentation, and compliance with care and regulatory standards.
- Collaborate with physicians, social workers, and payers to ensure appropriate resource utilization and avoidance of unnecessary readmissions.
- Lead and supervise care coordination teams to implement cost-effective, patient-centered care strategies.
- Identify barriers to care and resolve clinical or social issues proactively to facilitate timely discharge and follow-up.
Qualifications:
- Hold a valid CA RN license uploaded to Nursys within the last 2 weeks.
- Possess 5+ years of RN Case Management experience in an inpatient hospital setting.
- Demonstrate expertise in inpatient acute care case management and care coordination.
- Maintain current BLS certification; AHA or ARC certifications preferred.
- Ability to analyze complex clinical and psychosocial situations with tact and professionalism.
- Strong communication skills and ability to collaborate effectively across healthcare disciplines.
