**** Our Licensed Practical Nurses:
*** Provide compassionate and effective care and treatments to our residents.
* Work with a team of professionals to ensure the residents comprehensive needs are met.
* Review routines, calendar of events and resident needs
* Ensure medications and supplies are ordered, in stock and counted
* Ensure all charting and record keeping is accurate and complete
* Attend meetings, trainings
* Give a thorough report to oncoming shift.
* Updates 24 hour report.
* Calendars appropriately for upcoming needs.
* Promote the Mission, Vision and Values of Transforming Age, our
** Culture of Excellence,
** in all areas of responsibility.
* Performs other duties as assigned.
** What you need to succeed!
*** Must be at least 18 years of age.
* Cleared Criminal Record clearance through the Department of Social Services prior to presence in the community.
* Health screening and TB test results not older than 6 months.
* Current First Aid Training and Certifications, per Title 22 guidelines.
* A current nursing license issued by the State of California is required.
* Minimum of six months in long term care, home care or related field.
* Demonstrated competence in technical and manual skills according to the scope of practice for RN's/LVN's.
* Working knowledge of computers and typing, sufficient enough to be successful with computerized Point of Care and other systems.
* Excellent written and verbal communication skills.
* Knowledge of aging process and implications of aging in a social setting.
* Ability to communicate professionally with residents, families, peers and employees in a multi-cultural environment.
* Completion of a background check, drug screen, physical, fingerprint, and TB test may be required per state and local guidelines (Title 22).

* Physical Requirements:
