Housing Pathways Coordinator
The Housing Pathways Coordinator manages St. Mary's Center's Transitional Housing (TH) program and serves as the primary coordinator for participant progression from homelessness to permanent housing.
The position is responsible for Transitional Housing operations, waitlist management, participant intake and exits, housing readiness activities, hotel voucher coordination, move-in and move-out planning, and coordination with housing partners and service providers. The Housing Pathways Coordinator ensures participants are prepared for successful permanent housing placement and facilitates a structured transition to Tenancy Sustaining Services (TSS) upon housing placement.
The Housing Pathways Coordinator serves as the primary owner of participant movement through Transitional Housing and is responsible for maintaining a participant-centered, housing-focused approach that supports successful exits to permanent housing.
Maximum Caseload: 24 Transitional Housing Participants
Essential Duties and Responsibilities- Monitor occupancy and unit utilization.
- Coordinate participant intake, enrollment, and orientation.
- Conduct Transitional Housing eligibility review and placement activities.
- Manage Transitional Housing participant waitlists.
- Maintain participant files and required documentation.
- Coordinate participant exits and transitions.
- Ensure compliance with Transitional Housing program requirements including subsidies through Moving to Work or other programs
- Track participant length of stay and housing outcomes.
- Conduct house meetings weekly at each of the transitional housing sites.
- Provide training to Senior Housing Services staff on the Transitional Housing Program, including eligibility, referral process, program requirements, and client expectations.
- Develop individualized housing readiness plans.
- Assist participants with Housing Passport completion.
- Support document readiness and housing application preparation.
- Coordinate participant goal setting and housing planning activities.
- Monitor participant progression toward permanent housing.
- Collaborate with Housing Navigators regarding housing opportunities.
- Coordinate participant move-ins and move-outs.
- Develop move-in and transition timelines.
- Assist participants in preparing for permanent housing placement.
- Coordinate participant housing transition activities.
- Ensure participant records and transition documentation are complete.
- Coordinate with Housing Navigators, landlords, housing providers, and community partners.
- Coordinate security deposit requests and furniture services through Abode Services and other providers of flex funds
- Assist with housing placement logistics.
- Support participant housing interviews and inspections.
- Maintain communication with housing partners.
- Coordinate move-in basket essentials and welcome resources for newly housed participants.
- Prepare exit baskets and certificates.
- Coordinate warm handoff to tenancy sustaining services.
- Promote participant engagement and achievement recognition.
- Conduct structured participant handoffs to TSS Program Coordinator upon housing placement.
- Review participant needs, barriers, and support plans with TSS staff.
- Ensure continuity of care and service coordination.
- Transfer Housing Passport and housing transition documentation.
- Maintain timely HMIS and Salesforce documentation.
- Track housing outcomes and participant progression.
- Complete required program reports and documentation.
- Participate in quality assurance activities and case conferences.
- Transitional Housing occupancy maintained at target levels.
- Waitlists actively managed and updated.
- Participants progress toward permanent housing placement.
- Housing readiness plans maintained and updated.
- Move-ins and move-outs coordinated successfully.
- Housing outcomes tracked accurately.
- Timely and effective handoffs to TSS completed.
- Documentation standards maintained.
- Compliance requirements achieved.
- Participant satisfaction and engagement promoted.
- Bachelor's degree in Human Services, Social Work, Public Administration, or related field preferred, or three years' experience in transitional housing or property management
- Two years of experience in housing services, homelessness services, case management, or supportive services.
- Experience working with vulnerable populations.
- Strong organizational, coordination, and problem-solving skills.
- Ability to manage multiple priorities and deadlines.
- Valid California Driver's License.
- Experience managing Transitional Housing programs.
- Experience with Housing First and Coordinated Entry systems.
- Experience with Housing Authority and Moving to Work programs
- Experience with HMIS and Salesforce.
- Experience serving older adults experiencing homelessness.
- Knowledge of Alameda County housing resources and homeless response systems.
