We are seeking a compassionate Case Manager to support clients by connecting them with resources and developing service plans.

Responsibilities

+ Assess client needs

+ Develop and monitor case plans

+ Connect clients with community resources

+ Maintain case documentation

+ Conduct follow-up and outreach efforts

Requirements

Requirements

+ Bachelor's degree in Social Work, Human Services, or related field preferred

+ Case management experience

+ Strong interpersonal skills

+ Experience working with diverse populations
