Case Manager
Brief
Job Description:
The Case Manager works with clients to assess needs, develop service plans, coordinate resources, and monitor progress toward personal or program goals.
Responsibilities:
+ Conduct client assessments and develop individualized service plans
+ Connect clients with healthcare, housing, employment, or social services
+ Monitor progress and update case documentation regularly
+ Advocate for clients and coordinate with community partners
+ Maintain accurate records in compliance with program standards
+ Provide crisis intervention and follow-up support as needed
Requirements
Requirements:
+ Bachelor's degree in social work, counseling, human services, or related field preferred
+ Experience in case management or direct client services
+ Strong documentation and organizational skills
+ Ability to handle sensitive situations with professionalism and empathy
+ Knowledge of community resources and service coordination
