Certified Nursing Assistant (CNA) - Long Term Care

The Long Term Care CNA focuses on continuous, resident-centered care in skilled nursing facilities or extended care environments. You provide daily living support, monitoring, and companionship to residents with chronic conditions.

You work closely with nursing staff to maintain comfort, safety, and adherence to individualized care plans while documenting routine observations.

Responsibilities
  • Assist with activities of daily living and mobility for residents.
  • Monitor and document vitals, intake/output, and changes in condition.
  • Support with medication administration under supervision and with order verification as required by facility policies.
  • Assist with routine procedures and setup for therapies and activities.
  • Maintain a clean, safe, and respectful environment for residents.
  • Document care provided and report changes promptly to the care team.
  • Support residents' emotional well-being and provide basic education to residents and families.
Qualifications

Education:

High school diploma or GED plus completion of a state-approved CNA program with competency evaluation.

Licensure:

Current state CNA certification in good standing; CPR/BLS certification.

Certifications:

  • CNA certification
  • CPR/BLS certification

Experience or Skills:

Strong observational skills, communication, and ability to work as part of a care team in a long-term care setting.

Experience
  • Experience providing resident-focused care in SNF or long-term care settings.
  • Experience documenting care and vital signs in electronic records.
  • Experience collaborating with RNs, LPNs, and therapists to implement care plans.
Preferred
  • Experience with wound care or fall prevention programs
  • Knowledge of infection control practices
  • Spanish or other language skills
Work Environment

Long term care CNAs work in skilled nursing or assisted living facilities with resident-focused schedules. Shifts may include days, evenings, nights, and weekends. Documentation is performed in the facility's EMR, with emphasis on accuracy and timely reporting.

Team-based care with ongoing resident assessment, care planning, and collaboration with interdisciplinary teams.
