Role:

Licensed Practical Nurse (LPN) in a skilled nursing and post-acute rehab setting.

• Provide compassionate, resident-first care with hands-on clinical tasks such as medication administration, wound care, and vital sign monitoring.
• Support individualized rehabilitation and long-term care plans.
• Collaborate with an interdisciplinary team to achieve positive clinical outcomes.
• Ensure safety, dignity, and accurate documentation in the EHR.

• Licensed Practical Nurse licensure.
• Strong clinical skills in medication management, wound care, and vital signs.
• Ability to work collaboratively within a team.
• Excellent documentation and communication skills.
