As a Case Manager (Housing Navigation), you'll help Fairfax County families secure and maintain safe, affordable housing while building the skills and resources needed for long-term self-sufficiency. Working closely with participants, landlords, housing authorities, and community partners, you’ll guide families through housing searches, affordability planning, lease-up support, and service coordination to help them achieve stability and independence.

In this role, you will:
  • Assist participants with housing searches, relocation planning, and move-in coordination
  • Identify affordable housing opportunities and conduct landlord outreach
  • Provide ongoing case management, assessments, and goal planning
  • Connect families with community resources, benefits, employment supports, childcare, transportation, and education services
  • Coordinate with FCRHA, community partners, and internal team members to ensure participants receive comprehensive, wraparound support

This position is funded through a two-year contract. Service Source is committed to finding continued employment for staff should future funding or program needs evolve.

Primary Duties
  • Provide housing navigation and relocation support, including housing searches, affordability planning, move-in coordination, property identification, landlord outreach, and lease-up support.
  • Deliver participant-centered case management through assessments, monthly engagement, goal planning, and ongoing review of progress toward self-sufficiency.
  • Develop and update individualized, person-centered plans that address employment, housing stability, family support needs, and long-term independence.
  • Coordinate closely with FCRHA, housing navigators, Case Managers, EDSs, and community partners to align services with ITSP goals, MTW objectives, and wraparound support needs.
  • Connect participants to internal and external resources, referrals, training, benefits, childcare, transportation, employment, education, housing supports, and maintain accurate service documentation.
Additional Responsibilities
  • Build and maintain long-term relationships with community partners, participants, their families, and other stakeholders (e.g., medical teams, paid and natural supports, legal guardians) to ensure access to up-to-date resources and support.
  • Regularly communicate and collaborate with FCRHA support staff.
  • Stay informed on current and emerging community resources, including programs and services related to healthcare, housing, education, employment, and other relevant supports.
  • Build and maintain partnerships with local organizations to ensure access to the most up-to-date information, enabling effective referrals and support for individuals and families.
  • Record participants’ progress including charting referrals, visits, and other notable interactions
  • Facilitate and participate in team meetings.
  • Identify and/or provide support resources related, but not limited to independent living, aging in place, medical support, mental health support, emergency response plans/support, in-home companion care, housing, social and leisure activities, transportation, and financial planning and needs.
  • Actively seek opportunities to enhance knowledge of systems supporting FCRHA recipients, including the barriers they face. Foster collaboration through participation in community meetings, professional networks, and relevant trainings, enabling effective referrals and comprehensive care for individuals and families.
