Synergy Medical Group is a physician-owned statewide post-acute care group expanding across Florida. We're hiring physicians ready for a leadership-track role rounding at skilled nursing facilities (SNFs), assisted living facilities (ALFs), and inpatient rehab facilities (IRFs).

• Base range: percentage-of-collections | no income caps
• Top earners exceed $45,000/month in total compensation

• Round at partnered SNFs, ALFs, and IRFs in your region
• Manage medically complex adult patients with acute and chronic conditions
• Collaborate with NPs/PAs and facility interdisciplinary teams
• Practice independently with full administrative support behind you
• Option to grow into Medical Director or regional leadership roles

• Percentage-of-collections model - your income scales with your work, not a fixed RVU or salary ceiling

• MD or DO with an active Florida medical license (or eligibility); NP/PA applicants will not be reviewed for this role
• Experience managing medically complex adult patients
• Comfortable practicing independently
• Background in hospital medicine, internal medicine, family medicine or geriatrics
• Strong communicator with nursing and facility staff
