Position Overview - Step-Down Navigator

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes.

He/ She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Responsibilities include, but not limited to:

  • Meeting with eligible patients to discuss the benefits of step‑down services as part of their continuum of care treatment.
  • Communicating eligibility to senior leadership and associated departments.
  • Collecting and entering data specific to eligibility into Midas for trending report purposes.
  • Coordinating between departments to ensure a smooth transition for patients.

Qualifications

Requirements:

  • Education: High School graduation or GED required. Bachelor’s degree in social work or a clinical related mental health field, preferred.
  • Experience: Minimum two (2) years’ experience in an inpatient psychiatric/mental health setting.
  • Licensure: Current CPR, Physical and Verbal Crisis Intervention Certification. Training may be obtained during new hire orientation.
