Hospice of Marion County, part of the Empath Health family, is seeking a Care Navigation RN Facilitator to join our dedicated team. This role provides leadership and coordination for the Care Navigation team, ensuring excellent patient transitions, collaboration with community partners, and seamless access to Empath Health’s Integrated Network of Care.

Schedule:

Tuesday - Friday | 10 hour shifts, 40 hours a week

• Lead, support, and evaluate the Care Navigation team to ensure effective daily operations and outstanding patient service.
• Collaborate with out-of-area partners to facilitate smooth patient transfers and appropriate care coordination.
• Serve as a key resource and advocate for patients, families, and care partners throughout the admission and transition process.
• Partner with internal departments—including Finance, Reimbursement, and Interdisciplinary Teams—to ensure continuity of care.
• Maintain compliance with hospice regulations and documentation standards within the EMR.
• Participate in ongoing review and improvement of Care Navigation processes, promoting Empath Health’s mission and values.

• Minimum two (2) years of hospice experience strongly preferred.
• Valid Florida driver’s license and reliable transportation required.
• Strong communication, leadership, problem‑solving, and organizational skills.
• Proficiency in Microsoft Office and Electronic Medical Record (EMR) systems.
