As a Social Services Director at The Lodge Health & Rehabilitation Center, you will oversee the planning, development, and implementation of social service programs that address patient and family needs. This role ensures that patients and their families receive support addressing emotional, social, and practical needs during care, treatment, and recovery.
Key Responsibilities:Plan, develop, and oversee social service programs that address patient and family needs
Ensure integration of social services with overall patient care plans
Evaluate program effectiveness and implement improvements
Direct and supervise services such as counseling, discharge planning, and community resource referrals
Recruit, train, and supervise social work staff and interns
Ensure adherence to federal, state, and local healthcare and social work regulations
Work closely with physicians, nurses, and allied health professionals to support holistic care
Master's degree in Social Work (MSW) required;
Licensed Clinical Social Worker (LCSW) preferredCertification in healthcare or gerontology social work is a plus
Minimum 5 years of progressive social work experience in a healthcare setting
At least 2-3 years in a supervisory or leadership role
Strong leadership and team-building abilities
Excellent communication, counseling, and conflict-resolution skills
Knowledge of community resources, Medicare/Medicaid, and insurance processes
Ability to manage multiple priorities and work under pressure
Primarily based in a healthcare facility (hospital, long-term care, or rehabilitation center)
May require occasional travel to community agencies, outreach events, or training sessions
Full-time position, with potential for evening or weekend hours as needed
