APP Float - Primary Care+ Polk​/Lake

Advent Health Primary Care+ is seeking Float Advanced Practice Providers to join ourteam-based primary care model. This opportunity is designed for providers who value whole-family care, continuity with patients, and a collaborative practice environment that supports both high-quality care and work–life balance.

In this role, you will work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships.
