Home Health Occupational Therapist Full Time

Position Overview

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within home health care. You will collaborate with interdisciplinary teams, providing updates to enhance patient well-being.

• Conduct initial and ongoing assessments of mobility, strength, and functional levels.
• Administer, evaluate, and interpret tests to set functional goals and outcomes.
• Develop and document plans of care, including goals, progress, and discharge plans.
• Deliver occupational therapy services per physician orders.
• Maintain accurate, timely documentation to support care plans and ensure compliance.
• Coordinate discharge planning and educate patients and caregivers on home exercise routines.
• Collaborate with healthcare team members, physicians, and family members.
• Supervise other personnel such as OTAs and HHAs as applicable.
• Complete OASIS assessments where permitted by regulation.

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR, driver's license, and proof of auto insurance.

Preferred: Experience in home health therapy and OASIS documentation.
