Home Health PTA - Full Time

Role Overview: The Physical Therapy Assistant provides skilled education, treatment, and interventions under a physician's plan of care in a home health setting. They collaborate with an interdisciplinary team, update patients and caregivers, and ensure safe, effective therapy services.

Key Responsibilities:

• Deliver direct physical therapy following therapist and physician directives.
• Communicate regularly with patients and caregivers about care plans.
• Administer treatments, monitor patient progress, and report observations.
• Teach patients and caregivers safe device use and rehabilitation techniques.
• Coordinate with other disciplines and plan for patient discharge, including instructions.
• Participate in training programs, maintain confidentiality, and adhere to professional standards.
• Record patient progress, complete documentation timely, and ensure continuity of care.
• Maintain consistent attendance and submit credentials promptly.

Qualifications & Skills

• Graduate of an accredited Physical Therapist Assistant program.
• Current, unrestricted state license as a PTA.
• Valid CPR certification.
