Home Health Licensed Practical Nurse

Village Home Care is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our growing home health team. The LPN will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse and physician.

• Provide direct patient care in accordance with physician orders and care plans
• Monitor and document patient condition, vital signs, and progress
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and home care techniques
• Assist with wound care, catheter care, diabetic care, and other skilled nursing services
• Communicate changes in patient condition to supervising RN and physician
• Complete timely and accurate clinical documentation
• Participate in interdisciplinary team meetings and care coordination

• Minimum of 1 year nursing experience (home health experience preferred)
• Strong clinical, organizational, and communication skills
• Ability to work independently in a home setting
• Reliable transportation and valid driver’s license
• Current CPR certification
