Job Summary

The RN Case Manager (CM) is responsible for promoting patient-centered care by coordinating the plan of care for patients during their stay, managing length of stay, ensuring appropriate resource management, and developing safe discharge plans in collaboration with multidisciplinary teams.

Responsibilities
  • Provides case management services for both inpatient and observation patients.
  • Identifies patients at risk for adverse outcomes during transition between care levels.
  • Performs comprehensive psychosocial, medical, and discharge needs assessment of patients/family and evaluates resources appropriate and available.
  • Reassesses patient’s clinical condition as indicated, considers readmission risk, and develops strategies to mitigate risk through education and community resources.
  • Coordinates plan of care and drives discharge plan by collaborating with multidisciplinary team and patients’ physicians.
  • Works with Social Services to ensure post-acute medical needs and level of care are appropriate.
  • Refers to Social Services when psychosocial determinants of health risk factors are identified.
  • Involves patient, family, and significant others in clarifying needs and expectations to develop realistic goals.
  • Evaluates progression of care using evidence-based tools and approved criteria (Inter Qual) throughout the episode of care, escalating issues through the chain of command.
  • Makes appropriate referrals to third‑party payers, disease, and case‑management programs for recurring and chronic disease patients.
  • Facilitates patient throughput with focus on effective transition, quality, and efficiency.
  • Documents professional recommendations, discharge plan, care coordination interventions, and case‑management activities for team communication.
  • Aligns patient needs with available resources to ensure a safe discharge/transition.
  • Acts as liaison through professional communication between physicians, patients/families, staff, and outside agencies.
  • Seeks ways to control costs without compromising safety, quality, or services.
  • Directs activities to address needs of under‑resourced patients including education, assistance programs, and community resources.
  • Participates in performance improvement activities, identifying, documenting, and intervening when avoidable days occur.
  • Adheres to established policy, procedure, and standards of care; escalates issues timely.
  • Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives.
  • Advocates for patient rights, needs, and values, ensuring cultural and religious considerations are aligned.
  • Other duties as assigned.
Qualifications
  • Registered Nurse (RN) license.
  • Associate Degree in Nursing. Bachelor’s Degree preferred.
  • At least 2 years of acute hospital RN experience; preference for 4 years or more.
  • Certification in Case Management or Utilization Review is preferred.
  • No travel required.
