The Home Health Field Registered Nurse (RN) plays a critical role in delivering high-quality, patient-centered care within patients' homes. This role involves comprehensive assessment, planning, implementation, and evaluation of nursing care tailored to individual patient needs, often managing complex medical conditions. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote health, prevent complications, and support recovery or chronic disease management.

By providing education, emotional support, and clinical expertise, the RN empowers patients to achieve optimal health outcomes and maintain independence. This role ensures continuity of care outside traditional clinical settings, improving patient satisfaction and quality of life.

• Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs documented in clinical records, case conferences, team reports, evaluations, and activities of daily living (ADLs).
• Apply criteria for admission and re-certification to home health care to establish appropriate levels of care and patient eligibility.
• Develop, implement, and document the plan of care to ensure quality and continuity of care; revise the plan as necessary.
• Educate the patient and family regarding self-care techniques, nutrition, and dietary needs.
• Determine the scope and frequency of services based on acuity and patient/family needs; assess caregiver capacity to meet needs at admission and throughout care.
• Initiate preventative and rehabilitative nursing procedures as appropriate.
• Apply infection control concepts and standard precautions to prevent contamination and transmission of disease.
• Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service.
• Prepare clinical and progress notes that demonstrate progress toward established goals.
• Inform physicians and other personnel of changes in patient needs and intervention outcomes, while evaluating patient and family response to care.
• Monitor assigned cases to ensure compliance with third-party payor requirements.
• Maintain effective communication with patients, families, team members, and other health care professionals.

• Current RN license
• Valid driver’s license
• CPR certification
• Valid auto insurance
• At least 2 years of Home Health RN experience preferred
• Home Care Home Base experience preferred
