Hospice of Marion County, a member of Empath Health, is seeking a caring RN Case Manager to join the Care team, providing direct patient care in Marion County facilities.
Schedule:
Monday–Friday, daytime hours.
As RN Case Manager you will assess patients, coordinate with the interdisciplinary team, develop and update a Plan of Care, supervise LPNs and Home Health Aides, and administer medications as prescribed.
