Overview
The Integrated Care Manager at ESL is responsible for performing advanced indirect and selected direct client care duties in support of the clients’ living skills program in a competent manner under the direction of the Aftercare Program Supervisor. The Integrated Care Manager provides general oversight of the housing environment, reports all environmental needs as it relates to supporting clients to operations and the Aftercare Program Supervisor, and conducts case management duties including assessment, planning, and facilitation of care coordination to meet the client’s individual needs.
Additional responsibilities include assisting with transportation to appointments and providing medication management.

Tuesday – Friday 1:00 pm – 9:30 pm

Population ServedPersons meeting DSM‑5 criteria for substance use disorders with or without co‑occurring psychiatric illness, and psychiatric illness with or without substance use disorders.
ESSENTIAL DUTIES & RESPONSIBILITIES- Ensure all clients are woken up by 7:30 a.m.
- Facilitate morning meeting/meditation and chores
- Communicate with the MS Department and Directors of medication incident reports and med changes needed.
- Facilitate client transportation to CCI and outside appointments and activities in a timely manner.
- Conduct meal planning sessions.
- Manage weekly household expenses providing receipts and reporting remaining funds.
- Work with each client individually to assist them in attaining client treatment goals.
- Counsel clients as needed for behavioral issues and schedule emergency psychiatric care.
- Maintain and update Client Task Manager spreadsheet daily.
- Update Master Schedule Board and Client Information Station with schedule changes, weekly menu summary and chore list.
- Implement weekly UA test schedule and confirm.
- Oversee preparation for new clients in housing.
- Ensure clients are attending three outside meetings a week.
- Collect and review client Meeting Attendance Sheets, Phase‑up and Pass Requests.
- Schedule daily store‑runs for clients and manage cash and/or credit cards dispensed (as needed).
- Conduct Pass down with ICM staff using designated pass down template.
- Other duties as assigned.
- High school diploma or equivalent and one (1) year of experience in behavioral health care, preferred.
- Attainment of at least 18 years of age.
- Health screening with negative TB test results, current CPR/First Aid certification.
- Driver’s license – transportation of clients required; must have active driver’s license with a clean MVR.
- Experience as house manager or Mental Health Technician, preferably in the field of chemical dependency and/or behavioral health.
- Knowledge of industry standards as related to the position of the Integrated Care Manager.
- Basic knowledge of PC software programs, particularly Microsoft Word and Excel is preferred.
- Written and oral communication skills; must be able to concentrate on work amid distractions such as noise, conversations, and foot traffic.
- Must maintain self‑control in volatile or hostile situations such as when verbally or physically confronted in an aggressive manner.
- Must be able to work independently with little supervision in an unstructured environment with multiple demands.
- Must possess a reading and writing level that provides for clear expression and/or understanding of assignments related to the job description.
- Ability to represent the program well in interactions both inside and outside the facility.
