Home Health Registered Nurse

Overview

This is a full-time role for a Home Health and Hospice RN. We will train in-house through lots of shadowing with another RN. As a Home Health RN, you will play a critical role in providing excellent patient care in the comfort of their own homes. Your day-to-day tasks will include assessing patient needs, creating care plans, administering medications, monitoring vital signs, and providing wound care.

You will collaborate with a multidisciplinary team to ensure the holistic well-being of your patients and help them achieve their health goals.

• Valid RN license in the state of CA
• Minimum of 3 year of experience as a registered nurse is a plus
• Experience in home health or a related setting is preferred
• Hospice experience is a plus
• Excellent clinical assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and manage your time effectively
• Compassionate and empathetic approach to patient care
• Proficiency in electronic medical record (EMR) systems
• Current BLS certification
• Acute care clinical experience is a plus
