Home Care Physical Therapist-Per Diem (South Shore, Nassau County)
Part-time •
Compensation: USD 65 - USD 80 - hourly

Metro is looking for a motivated and passionate PER DIEM Physical Therapist to provide in-home care. - Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible
- Mandatory
Certifications:
First Aid, AED, & CPR Certifications - Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance
- Ability to travel to patients’ homes within the assigned geographic territory
The anticipated pay range for this position is $65-80 per visit
