Responsibilities
  • Accept and treat patients within your designated geographic area
  • Provide 1:1 treatment
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
  • Educate family members, home health aides, and other caregivers on proper therapy techniques
  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients
  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention
  • Provide timely and accurate documentation for each visit
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
  • Must be willing to treat geriatric patients
Qualifications
  • Occupational Therapist Program Graduate
  • Licensed Occupational Therapist in the state of New York or eligible
  • Mandatory

    Certifications:

    First Aid, AED, & CPR Certifications
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance
  • Ability to travel to patients’ homes within the assigned geographic territory
  • Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc
Additional Information

The anticipated pay range for this position is $65-80 per visit
. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

Compensation: $65-$65
