Description
Part of the Community Health Center (CHC) king with Inpatient and Outpatient individuals in a variety of tasks, including DSMT, GDM, Lab Draws, Injection Administration, and Infusions. RN will perform Diabetes Education to Inpatients, Outpatients and Telemedicine patients. RN will promote self-management skills for the patient to develop independence by establishing patient care goals and teaching patients and families to understand conditions, medications, and self-care skills.
CHC performs a variety of tasks including scheduled infusions and walk-in lab draws and injections (TDap, Rho Gam, Betamethasone, etc.). A wide degree of creativity and latitude is expected. Excellent skills in charting notes according to our ADCES Accreditation requirements is a must. High Degree of interpersonal care skills and compassion are required. High level of organization skills and self-motivation is expected.
RN reports to the Clinical Coordinator of CHC. Requires an associate's degree and is certified as a registered nurse. Prefer bachelor's degree in nursing. Prefer Certified Diabetes Educator (CDE), absent of that, will allow working towards CDE, to obtain within 2 years of hire date. Full bilingual skills in English/Spanish is required. You will work with an amazing team that is professional and respects each other.
We work for the good of the patient outcome and experience. CHC is open Monday - Thursday, 8:00am - 5:00pm; and Friday 8:00am - 12:00pm.
