As a Weekend Home Health RN
, you will provide skilled nursing care to patients in the comfort of their homes, ensuring continuity of care during the critical weekend window.

Conducting patient assessments and developing individualized care plans.

Providing skilled nursing care and essential patient/caregiver education.

Coordinating care with physicians and interdisciplinary teams.

Monitoring progress and adjusting care for patients recovering at home.

Documenting visits efficiently using our streamlined AI-assisted tools.
