JOB SUMMARY

Help families achieve self-sufficiency by conducting thorough interviews and evaluations to identify client needs and determine eligibility for the Case Management program. Design and implement service plans, modifying them as client reaches goals. Connect clients with external services. Track client progress, verify attendance, and intervene to remove barriers. Maintain accurate, confidential client records, log events, and document progress.

TASKS AND DUTIES (Essential Functions):
  • Conduct assessments, referrals, and provide direct assistance as appropriate.
  • Network with all service providers in the community.
  • Provide follow‑up visits and phone calls on a weekly basis.
  • Make home visits when necessary.
  • Complete program and progress reports as required.
  • Travel in Service Area up to 75% of the time
  • Attendance is mandatory
  • MINIMUM REQUIREMENTS (MANDATORY FOR HIRING):
  • Employee must have a High School diploma or GED.
  • The ability to work well with resource agencies, clients and other staff.
  • Must have knowledge of community resources.
  • Must have good communication skills
  • Dress appropriately for the job.
  • Must have computer experience
  • Must have a valid driver’s license.
  • Must have knowledge of office equipment

    The employee is often exposed to outside weather conditions and the noise level in the work environment is usually low.
