The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service, and the maintenance of an environment committed to serving the veteran. The LPN/LVN also performs assigned duties in a manner that promotes quality health care, outstanding customer service, and the maintenance of an environment committed to serving the veteran.
The LPN/LVN performs within this functional statement under guidance of the Nurse Manager or designee. The LPN is accountable for all patient care given and function within the LPN scope of practice as it relates to policy and procedures. The LPN is responsible for observing, recording and reporting changes in the patient's condition as well as assisting in the development and revision of patient driven holistic life care plan.
The LPN will initiate appropriate actions based on the treatment plan and their observation of patient condition under the supervision of RN Care Manager. LPN demonstrates active collaboration and communication with the patient care team (Provider, RN Care Manager, NA and MSA) and expanded team. The LPN assists with providing patient and family health education with focus on self-management, prevention and wellness.
