As a Social Worker in Case Management at ourfacilityyou will be joining a team that hasa passion for creating positive patient interactions and a dedication to caring for the multi-faceted wellness of those we serve. In this role you will:

• Be responsible forthe day-to-day direction of socialservicepatient care, including quality care of the patient from admission to discharge.
• Evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
• Act as a liaison between the facility and resources external to the organization.
• Implement timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.
• Provide crisis intervention and support.
• Assist the Case Manager in facilitating patient movement across the continuum of care and identify and track barriers to patient throughput.
• Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.

• Bachelor's degree in social work required; master's degree preferred.
• One of the following current State of Utah Social Worker licenses:
  Social Service Worker (SSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW).
• 2 or more years of clinical experience preferred.
• Knowledge of Utilization Review, DRG review, and Discharge Planning preferred.
