Job SummaryThis position provides primary care and treatment to a group of patients in a clinic or hospital unit. This position may be required to access and administer medications within their scope of practice and according to State Law.
Responsibilities- Assesses the physical condition of patients by performing physical examinations and obtaining medical histories.
- Formulates medical plans and prognosis based on patient condition.
- Consults with physicians regarding patient health care plans and treatment.
- Orders, interprets, and evaluates diagnostic tests to identify and assess patient clinical problems and health care needs.
- Performs selected invasive procedures for therapeutic and/or diagnostic reasons within scope of validated competence.
- Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy.
- Complies with medical records policies regarding the complete and accurate documentation of patient care; initiates and maintains required records and legal documents.
- Participates in quality improvement activities.
- Counsels patients, families and significant others about health and illness, and promotes health maintenance.
- Evaluates, plans, implements, and re-evaluates the care of individuals requiring emergency measures.
- Demonstrates clinical competence and in-depth knowledge of specialty area.
- Demonstrates leadership, teaching, research and communication skills.
- Provides care appropriate to the population served.
- Required: Licensure to practice as a Nurse Practitioner in the State of Utah.
- National certification as a Nurse Practitioner required by state law.
- Equivalency:
Master’s degree in Nursing or current licensure as a Nurse Practitioner. - Utah State prescriptive privileges.
- Certification in ACLS / BLS / NRP / ATLS may be required by department or privileging criteria.
- Immunization per CDC standards and hospital policy (exceptions for documented medical contraindications or religious beliefs).
- Consent to comply with required criminal background check and/or drug screen.
- Certification in ACLS, BLS, NRP, ATLS.
- Experience in palliative care and related services.
- Experience in teaching and academic program development.
- Documentation of medical services required by regulatory agencies.
- Direct Patient Care: Perform palliative care consults in hospital and outpatient settings; coordinate palliative care services; conduct routine, urgent, and emergent medical evaluations; prescribe medications; coordinate consultations; participate in discharge planning for inpatients with multiple active medical conditions; collaborate with faculty attending physicians and interprofessional teams.
- Teaching Duties: Assist with development and presentation of palliative care educational programs; collaborate with clinical departments; serve as clinical preceptor for APRN/PA students enrolled in approved degree programs.
- Documentation of Medical Services: Complete written documentation required by regulatory agencies; direct completion of POLST forms for appropriate patients.
