The Care Management Assistant provides administrative and clerical support to the Care Management Department. This role supports discharge planning activities, assists with daily office operations, greets visitors, and communicates with internal and external stakeholders. The position requires professionalism, sound judgment, flexibility, and strict adherence to confidentiality standards. This role does not provide direct patient care.

• Provide clerical and administrative support to care management staff
• Assist with discharge planning documentation and coordination
• Answer phones, route calls, and respond to inquiries
• Greet visitors and support departmental operations
• Maintain accurate records and confidential information
• Collaborate with team members in a fast-paced healthcare environment

Education: High School Diploma or GED required. Associate’s degree in a health‑related field preferred.

Experience: 1–2 years clerical experience in a healthcare setting, call center, or patient access.
