• Complete admissions, conduct social services evaluations, provide referrals, and maintain communication with residents and families.
• Assist guests with financial and legal matters (e.g., pensions, legal referrals, funeral planning).
• Provide discharge planning, including arranging community congregate living, home care intake, transfer arrangements, and coordination with outside agencies for safe discharges.
• Provide or arrange counseling and psychiatric medication management as indicated.
• Ensure accurate and timely documentation in MDS care plans.

• Preferred:
  At least one year of supervised social work experience in a long‑term health care setting.
