Responsibilities:

• Provide skilled nursing care to patients in their homes, focusing on prevention of illness, restoration of health, and promotion of wellness

• Assess patient needs, develop individualized care plans, and implement interventions to achieve optimal health outcomes

• Collaborate with other healthcare professionals, patients, and families to ensure comprehensive care

• Administer medications, perform wound care, and provide other medical treatments as needed

• Maintain accurate and detailed records of patient care and services provided

• Communicate with patients, families, and healthcare providers to ensure seamless continuity of care

Requirements:

• Current LPN licensure in the state of Ohio

• 1 year of experience in home health or a related field

• Strong assessment, planning, and implementation skills

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team

• Willingness to work flexible schedules, including weekends and evenings, as needed
