United Health Group - - Responsibilities:
Follow state home-care laws and the established plan of care as defined by supervising Physical Therapist;
Instruct and assist patients with active and passive exercises, gait, functional activities, ADL, transfers, safety, and prosthetic training;
Observe, record, and report patient responses and changes to supervising PT/nurse/physician;
Coordinate care with other healthcare team members;
Instruct patient, family, caregiver, and other health-care team members in therapy within PTA scope
