Location & ScheduleField-based throughout Greater Northeast Ohio | Full-Time | Monday – Friday
What You'll DoAs a Community Engagement Specialist, you'll serve as a non-clinical member of VNA's Hospice team - spending time with hospice patients in assisted living facilities, SNFs, and senior living communities, while building the relationships that make VNA Hospice the trusted partner of choice in Northeast Ohio.
With patients, you'll:
- Provide companionship and emotional support, especially for patients with limited family visitors
- Sit with patients at meals, encourage activity participation, and be a consistent, reassuring presence
- Observe and report non-clinical concerns (changes in mood, social isolation, unmet needs) to the VNA Hospice team
With facility staff, you'll:
- Build genuine, lasting relationships with administrators, nurses, social workers, activities coordinators, and CNAs
- Make VNA Hospice a visible, valued, and trusted presence within every facility you serve
- Identify and communicate referral opportunities to the Hospice and Business Development teams
This role does not include administering medications, performing clinical assessments, hands‑on personal care, or any task within the scope of a licensed clinical professional.
RequirementsMust-Have:
- High school diploma or equivalent
- Valid Ohio driver's license and reliable transportation
- Genuine warmth, patience, and comfort working with elderly or seriously ill patients
- Dependable and consistent - patients and facilities will count on you to show up
- Basic proficiency with email, scheduling tools, and reporting systems
Preferred / Nice-to-Have:
- Prior experience in caregiving, companion, volunteer, customer service, or community outreach roles
- Familiarity with hospice, senior care, or long-term care environments
- Coursework or experience in health, human services, social work, or a related field
