Occupational Therapist/ Home Care Case Manager

Salaried Position

• The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences.
• When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services.
• The Occupational Therapist provides care as ordered by the physician, follows policies, procedures, and conditions of participation, and regularly reports to their supervisor.
• The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.
• The incumbent is responsible for all required documentation related to the care provided and communicates pertinent information regarding the patient to all appropriate health care team members.

  Bachelor's degree

    Bachelors - Graduate of an accredited Occupational Therapy Program
  • Area of study or major:
    Occupational Therapy

• Current licensure to practice Occupational Therapy in Ohio
• CPR Certification

• Minimum Level of Experience
  
  Required:
  
  1 - 3 years of job-related experience
• Prior job title or occupational experience:
  Clinical experience should have occurred within the last three years unless currently employed in home care.
• Preferred experience:
  
  One year clinical experience in acute care setting preferred

• Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.
• Must be able to creatively adapt multiple environments and situations for successful outcomes.
• Must be self-directed and able to work with little supervision.
• Computer skills preferred.
• Home care experience preferred.
