Aventura at Walton Hills is seeking a compassionate, motivated, and licensed Physical Therapist Assistant (PTA) to join our full-time, in-house rehabilitation team. If you're passionate about helping residents achieve their highest level of independence while working in a supportive Skilled Nursing and Long-Term Care environment, we'd love to hear from you!
Position Highlights

- Skilled Nursing & Long-Term Care Setting
- Collaborative Interdisciplinary Team
- Utilize Net Health electronic documentation system
- Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
- Assist residents in improving mobility, strength, balance, endurance, and functional independence.
- Accurately document treatments, resident progress, and outcomes using Net Health.
- Collaborate with physicians, nursing staff, therapy colleagues, and interdisciplinary team members to deliver exceptional resident-centered care.
- Maintain compliance with all federal, state, facility, and professional practice standards.
- Educate residents and families on treatment plans, safety techniques, and home exercise programs as appropriate.
- Current Ohio Physical Therapist Assistant (PTA) license in good standing.
- Skilled Nursing Facility (SNF) or Long-Term Care experience preferred.
- Experience with Net Health preferred.
- Excellent communication, organizational, and teamwork skills.
