The Therapist provides professional behavioral health services to individuals, families, and/or groups experiencing mental health, emotional, behavioral, trauma‑related, or substance use concerns. The Therapist is responsible for completing assessments, developing individualized treatment plans, providing counseling services, documenting clinical care, and coordinating services in accordance with agency policies, Medicaid requirements, and applicable state and federal regulations.
This position requires an active LSW license, National Provider Identifier, and Medicaid provider number to support direct billing and service delivery.
Essential Duties and Responsibilities- Provide individual, group, and family counseling services within the scope of an LSW license.
- Complete diagnostic assessments, screenings, psychosocial assessments, and clinical updates as assigned.
- Develop individualized treatment plans based on client needs, strengths, diagnoses, goals, and medical necessity.
- Provide trauma‑informed, culturally responsive, and client‑centered therapeutic interventions.
- Maintain accurate, timely, and clinically appropriate documentation in the electronic health record.
- Document progress notes, treatment plans, assessments, discharge summaries, and other required clinical forms according to Medicaid and agency standards.
- Maintain an active caseload and meet productivity expectations established by the agency.
- Coordinate care with internal team members, external providers, schools, courts, child welfare, primary care, or other community partners as appropriate.
- Participate in clinical supervision, team meetings, case consultations, and ongoing professional development.
- Monitor client progress and adjust treatment plans as clinically indicated.
- Complete risk assessments, safety planning, crisis intervention, and mandated reporting when necessary.
- Maintain compliance with HIPAA, confidentiality requirements, ethical standards, and professional boundaries.
- Ensure services are provided in accordance with Medicaid billing standards, agency policies, and applicable licensing regulations.
- Maintain active licensure, NPI, Medicaid provider number, and required training credentials.
- Current and active Licensed Social Worker license in good standing.
- Active National Provider Identifier.
- Active Medicaid provider number or ability to maintain Medicaid enrollment.
- Bachelor’s or Master’s degree in Social Work from an accredited program, depending on agency and program requirements.
- Experience providing behavioral health, mental health, substance use, or trauma‑informed services preferred.
- Knowledge of Medicaid documentation and medical necessity standards preferred.
- Ability to complete clinical documentation accurately and on time.
- Ability to work with diverse populations, including children, adolescents, adults, and families.
- Strong communication, organization, and ethical decision‑making skills.
- Experience working in community behavioral health.
- Experience with clients impacted by trauma, substance use, poverty, child welfare involvement, court involvement, or serious emotional disturbance.
- Familiarity with electronic health record systems.
- Training in CBT, DBT skills, motivational interviewing, trauma‑informed care, family systems, or other evidence‑based practices.
- Experience facilitating groups.
The Therapist will provide services within the legal and ethical scope of practice for a Licensed Social Worker. Services may include counseling, case consultation, assessment, treatment planning, care coordination, and crisis support. Clinical supervision will be provided as required by agency policy, payer standards, and licensing requirements.
