Caregiver DSP

As a Caregiver DSP at Sevita, you'll provide person-centered support to individuals with diverse needs in home and community settings. Responsibilities include assisting with daily living activities, personal care, meal preparation, medication reminders, transportation, and community engagement. You'll document care, follow individualized service plans, and communicate with families and clinical teams. Responsibilities

• Assist individuals with daily living tasks including bathing, dressing, grooming, and toileting
• Support meal planning, preparation, and feeding as needed
• Provide transportation and support for appointments, activities, and community outings
• Follow individualized service plans and behavior support strategies
• Offer companionship, encouragement, and skill-building for greater independence
• Monitor and report changes in health, behavior, or needs to supervisors and families
• Complete accurate, timely documentation of services and incidents
• Maintain a safe, clean, and respectful home environment
• Follow infection control, safety, and emergency procedures
• Participate in required training, coaching, and team meetings
