Direct Support Professional​/Caregiver

Join Sevita as a Direct Support Professional/Caregiver in our Healthcare & Medical Services, Community & Social Services team. In this person-centered role, you'll support individuals with developmental, intellectual, or physical disabilities in home and community settings. Assist with daily living activities, medication reminders, transportation, and community engagement while promoting independence, safety, and dignity. You'll follow individualized service plans, document care, and collaborate with families and clinicians.

Responsibilities

• Provide direct care and support in home and community settings
• Assist with activities of daily living such as bathing, dressing, and meals
• Offer medication reminders per care plans and document services accurately
• Support community integration, social activities, and skill-building
• Promote safety, dignity, and independence for each person served
• Follow individualized service plans and clinical guidance
• Communicate effectively with families, supervisors, and care teams
• Observe and report changes in physical or behavioral status
• Maintain a clean, safe, and respectful environment
• Participate in ongoing training and professional development
