Direct Support Professional

Sevita is seeking a Direct Support Professional for our Day Program, supporting individuals with diverse abilities in a community-based setting. You'll provide person-centered care, assisting with daily living skills, personal care, and behavioral support while promoting independence and dignity. In this role, you'll lead and participate in engaging activities, accompany individuals into the community, and ensure a safe, respectful environment. Responsibilities

• Support individuals in daily living skills, personal care, and hygiene as needed
• Implement person-centered service plans and behavior support strategies
• Supervise and engage individuals in day program and community activities
• Monitor individuals' safety and well-being, following all care protocols
• Accurately document services, behaviors, and progress notes
• Assist with transportation and mobility support as required
• Communicate effectively with team members, families, and supervisors
• Maintain a clean, safe, and respectful program environment
