Professional Case Management, a leader in in-home healthcare for individuals with complex medical needs, is seeking a compassionate Home Health Aide. In this role, you will provide personalized care and daily living support to clients, including former energy workers and their families, in the comfort of their homes. Responsibilities include assisting with personal care, monitoring client conditions, and supporting independence and safety.

Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and toileting
- Assist clients with safe ambulation, transfers, and positioning according to care plans
- Support activities of daily living such as meal preparation, feeding assistance, and light housekeeping
- Observe and promptly report changes in client condition, behavior, or safety concerns to supervising clinicians
- Follow individualized care plans and instructions from nurses and other licensed professionals
- Offer companionship, emotional support, and encouragement to promote client independence and well-being
- Assist with use of mobility aids and basic medical equipment as directed
- Provide medication reminders consistent with care plan and company policy
- Maintain accurate, timely documentation of care provided and client status
- Comply with infection control, safety, and confidentiality policies at all times
- Communicate effectively with clients, families, and the healthcare team to coordinate care
Required Skills
- Personal care assistance (bathing, grooming, dressing, toileting)
- Safe patient handling, transfers, and mobility support
- Basic knowledge of vital signs observation and reporting
- Home safety and infection control practices
- Use of mobility aids and simple medical equipment (e.g., walkers, wheelchairs, oxygen support under direction)
- Meal preparation and safe food handling
- Accurate documentation and basic charting
- Time management and route planning for home visits
- Clear verbal and written communication
- Basic computer or mobile app use for electronic charting
