Description

Inniswood Health & Rehab is seeking a compassionate, dedicated, and organized Social Services Designee to join our team. This position plays an important role in supporting residents and their families while helping ensure their psychosocial, emotional, and social needs are met.

Position Responsibilities:

  • Assist residents and families with social service needs and concerns.
  • Provide support during admissions, transitions, and discharge planning.
  • Develop positive relationships with residents, families, and the interdisciplinary care team.
  • Participate in care plan meetings and assist with identifying resident needs and goals.
  • Help connect residents and families with appropriate community resources.
  • Provide emotional support and advocacy for residents.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Assist with resident rights, grievances, and quality-of-life initiatives.
  • Coordinate with nursing, therapy, activities, and other departments to promote comprehensive resident care.
  • Maintain confidentiality and treat every resident with dignity, respect, and compassion.

Qualifications:

  • Experience in a healthcare, long-term care, social services, or senior living environment preferred.
  • Strong communication, organization, and interpersonal skills.
  • Compassionate and resident-focused approach.
  • Ability to work effectively with residents, families, and an interdisciplinary team.
  • Knowledge of long-term care regulations and social services practices is a plus.
