WHAT YOU’LL DO

As a Physical Therapist, you will provide high-quality therapy services to our residents while helping them achieve their highest possible level of function.

Responsibilities include:

  • Evaluate residents and develop individualized treatment plans

  • Provide skilled physical therapy services in accordance with physician orders and established care plans

  • Perform therapeutic exercises and functional activities

  • Assist residents with improving strength, balance, coordination, mobility, and endurance

  • Provide gait training and transfer training

  • Recommend appropriate assistive devices and adaptive equipment

  • Monitor resident progress and adjust treatment plans as appropriate

  • Document evaluations, treatments, progress, and discharge information accurately

  • Communicate effectively with nursing, physicians, families, and other members of the interdisciplinary team

  • Participate in care planning and interdisciplinary meetings as appropriate

  • Educate residents, families, and caregivers regarding therapy programs and safe mobility

  • Follow all applicable federal, state, and facility regulations and professional standards

  • Maintain a safe, respectful, and encouraging environment for residents

THE IDEAL CANDIDATE

We are looking for a Physical Therapist who is:

  • Compassionate and patient-focused

  • Dedicated to improving residents' quality of life

  • Professional and dependable

  • A strong communicator

  • Comfortable working with an interdisciplinary healthcare team

  • Organized and detail-oriented

  • Able to work independently while also being a strong team player

  • Passionate about rehabilitation and helping people reach their goals

Experience in skilled nursing, long-term care, rehabilitation, geriatrics, or a similar setting is a plus!

QUALIFICATIONS
  • Current Physical Therapist license in the applicable state

  • Graduate of an accredited Physical Therapy program

  • Current CPR certification or ability to obtain

  • Strong clinical and documentation skills

  • Ability to work effectively with residents, families, and healthcare professionals

PRN / FLEXIBLE OPPORTUNITIES AVAILABLE

We are also open to PRN Physical Therapists!
