Advantage Care Rehabilitation is looking for a PRN Physical Therapist (PT) to join our rehabilitation team at Grand Rapids Care Center in Grand Rapids, Ohio

What Will You Do as a Physical Therapist?As a Physical Therapist at Grand Rapids Care Center
, you'll evaluate and treat patients with a variety of mobility and functional needs.
Your role may include:
- Performing comprehensive Physical Therapy evaluations
- Developing individualized plans of care based on patient needs and goals
- Addressing strength, balance, gait, transfers, endurance and functional mobility
- Providing therapeutic exercise, neuromuscular re-education and functional mobility training
- Assessing the need for assistive devices and providing appropriate training
- Developing fall-prevention and safety strategies
- Monitoring patient progress and modifying treatment plans when appropriate
- Educating patients, families and caregivers
- Collaborating with PTAs and the interdisciplinary care team
- Completing accurate and timely clinical documentation
- Helping patients work toward greater safety, mobility and independence
