Your role as a Registered Nurse Case Manager (RN)

As a PRN Registered Nurse Case Manager (RN) at VNA of Ohio Behavioral Health Operations, LLC
, your day-to-day expectations will include conducting comprehensive patient assessments and developing individualized care plans that address the specific needs of each patient. You will be responsible for coordinating services with an interdisciplinary team, ensuring that all aspects of care are efficiently managed. Daily tasks will involve monitoring patient progress, conducting home visits, and providing education to patients and their families about health management and resources available to them.

Additionally, you will maintain accurate documentation and communication with healthcare providers to facilitate continuity of care. What you need to be successful:
