Key Responsibilities- Conduct thorough annual home health risk assessments, averaging 45 minutes per visit
- Evaluate patients medical history, current medical conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and offer recommendations to improve patient outcomes
- Visits may also include post‑discharge assessments
- No requirement to create treatment plans or prescribe medications
- Active Nurse Practitioner license in Ohio
- Current BLS/CPR certification
- Board Certification from AANP or ANCC
- Preferred minimum of six months recent NP experience
- MSN degree from an accredited nursing program
