Nurse Practitioner - State Traveler

Overview

$7,500 Sign-On Bonus. Matrix Medical Network provides in-home clinical assessments to improve health and care for older adults and at-risk individuals.

During a visit that lasts up to one hour, providers review and observe a member’s health status, medical history, medication adherence, social environment, and other risks. This assessment informs a comprehensive health plan and helps close care gaps. Key tasks include:

• Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, and referrals.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, caregivers, and ancillary medical personnel.
• Coordinate follow‑up with the Primary Care Physician (PCP) for patient education and ongoing care.
• Provide services at the member’s residence in homes or skilled nursing facilities.

• Consistent schedule:
  Full‑time providers work Monday to Friday, 8:00 a.m.–5:30 p.m.; assessments are available Sunday–Saturday, 8 a.m.–7 p.m.
• Competitive base salary: $105,500 – $168,400 annually, based on market analysis and experience.
• Incentive program:
  Eligible for a monthly (uncapped) incentive; average $500 per month ($6,000 annually).

• Educational support:
  Comprehensive onboarding, preceptor assignment, 1:1 feedback, free access to continuing education.
• Licensure support:
  Assistance obtaining additional state licensure and credentialing in neighboring states as needed.

• Master’s Degree or equivalent experience plus successful completion of NP licensure.
• Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority.
• Board certification by AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
• Current BLS, ACLS or CPR certification.
• One year of NP experience.
• Willingness to travel up to 80‑90% of the time, including overnight stays and weekends across the assigned state and neighboring states.
• Valid driver’s license for the state of residence and personal automobile insurance coverage required.
• Liability insurance coverage:
  Bodily injury $100,000 per person/$300,000 per incident;
  Property damage $50,000;
  Collision and comprehensive deductibles $1,000 or less.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
