DescriptionCentral Ohio Primary Care is seeking a full‑time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join Family Practice Center of Westerville. The APP will work collaboratively with physicians and clinical staff to provide high‑quality, patient‑centered primary care for patients of all ages. Responsibilities include assessment, diagnosis, treatment planning, chronic disease management, preventive care, and patient education in a busy outpatient family medicine setting.

- Monday - Thursday: 7:00 AM – 4:00 PM
- Friday: 7:00 AM – 3:30 PM
- Perform comprehensive patient evaluations including medical histories, physical examinations, and diagnostic assessments
- Diagnose and manage acute and chronic medical conditions across the lifespan
- Provide preventive care services including annual wellness visits, screenings, immunizations, and health counseling
- Develop and manage treatment plans for chronic diseases such as diabetes, hypertension, asthma, COPD, and hyperlipidemia
- Prescribe medications and monitor treatment effectiveness in accordance with Ohio licensure and practice guidelines
- Educate patients and families on disease prevention, treatment options, medications, and healthy lifestyle strategies
- Collaborate with physicians, nursing staff, and specialists to coordinate comprehensive patient care
- Facilitate referrals, follow‑up care, and transitions of care as needed
- Maintain accurate and timely documentation within the electronic health record system (EPIC)
- Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
- Current Ohio licensure as a Nurse Practitioner or Physician Assistant National certification as a CNP or PA-C
- Minimum of one year of post‑graduate clinical experience preferred, ideally in outpatient primary care or family medicine
- Strong clinical assessment, diagnostic, and patient management skills
- Excellent communication and interpersonal abilities
- Ability to work independently while collaborating effectively within a multidisciplinary team
- Experience with EPIC EHR preferred, though training will be provided
