Job Title:
Home Health RN Case Manager & Recertification Nurse (RNCM)
Road Warrior leadership position requiring travel as needed to all clients throughout Northwest Ohio. Works closely with and reports directly to the NW OH Regional Program Administrator.
Work Consists Of- Case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
- New hire orientations, in‑service and supervisory reviews, and input to influence training, education, care practices, outcomes, and overall business success.
- Back‑up and occasional Skilled Nursing visits and services including med sets and dressing changes.
- At least 1 year of experience in case management, case admissions and discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and Non‑OASIS assessments, and excellent documentation and field staff leadership.
- At least 1 year of paid experience with a home health agency or equivalent; 2–5 years preferred.
- Nurse supervision experience.
- People management.
- Peer/staff education and development.
- Success or willingness to build relationships with client payors, case managers, and entity relationships (e.g., AAA, DoDD County Board, VA) in NW Ohio.
- Road Warrior position covering the entire Northwest Ohio region:
Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas. - Willingness to travel throughout this region is required.
- Daytime availability (8 am–5 pm) with occasional evening availability; no weekends required.
- Long‑term, full‑time RNCM/Recertification Nurses.
- No on‑call or overnight travel required.
- Weekend services are optional.
- Round‑trip/Door‑to‑Door mileage reimbursement.

- Full‑time annual salary plus PTO and mileage: $78,000–$83,200 per year, based on minimum 2‑hour travel radius and required average visits per week, plus quarterly bonus opportunities.
As an RN Recertification Nurse, you will coordinate, re‑evaluate, and confirm the medical necessity of continued home health care services, provide patient care, and ensure optimal outcomes.
- Oversee patient services and provide expert nursing care with core skills in managed care, patient assessments, compliance, treatment plan documentation, and discharge planning.
- Submit updated documentation of patient condition and care needs timely and within compliance of all state, federal, and agency guidelines.
- Ensure successful communication with physicians, agency staff, service administrators, other caregivers and clinicians, and client families.
- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
- Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Document, communicate, and coordinate patient status or changes in condition to the care team.
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Willing to travel surrounding counties.
- Minimum of one year of nursing supervisory experience; home care experience preferred.
Regional Program Administrator, or Clinical Administrator/Director of Nursing.
Qualifications- Graduate of an accredited School of Nursing.
- Minimum of 1 year of experience as a Registered Nurse, with an active RN license to practice in Ohio.
- Minimum of 1 year of experience in a home health agency or equivalent setting.
- CPR and First Aid certifications required (or can be obtained prior to hire).
