Shift AvailableFull-Time Night Shift: 7:00 PM – 7:00 AM
What You Will Do- Provide direct resident care services while following individualized plans of care for all residents.
- Administer medications in accordance with all standards of practice, state regulations, and community policies, including required documentation, reporting, and follow-up.
- Perform resident assessments as required by state regulations and community policies under the direction of the Director of Assisted Living.
- Observe residents for changes in condition and immediately report concerns to the supervisor and/or Director of Assisted Living.
- Document all pertinent observations, communication with residents' responsible parties, clinical procedures, and daily care activities by the end of each shift.
- Complete and maintain individualized care plans for residents.
- Assist with overnight resident monitoring, wellness checks, and emergency response as needed.
- Perform other duties as assigned by leadership.
This position works under the supervision of the Director of Assisted Living and/or Director of Memory Care but is expected to perform independently and exercise sound judgment. The position may assist with supervision, direction, and review of other associates' work and may participate in performance evaluations and disciplinary actions as needed.
Why You Are QualifiedThe ideal candidate will have:
- High school diploma or equivalent (GED) required.
- Current Licensed Practical Nurse (LPN) license in the State of Ohio required.
- Current CPR certification required.
- Minimum of one (1) year of related nursing experience preferred.
- Excellent communication, documentation, and resident care skills.
- Ability to work independently during overnight hours while maintaining a high level of resident care and safety.
On a daily basis, throughout the workday, must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching, kneeling, reaching overhead, grasping with both hands, and performing fine motor tasks. Must be able to lift up to 25 pounds unassisted and assist with carrying, transferring, repositioning, pushing, and pulling residents using proper equipment and assistance when necessary.
